Just wanted to bring closure to this thread by posting the solution.

As advised I had the missing exhaust gaskets installed and it passed the test this time round. I also had a new fuel filter installed while they were under there and new fuel filter + gaskets = wow (all the horses are back in the stable)

Many thanks for all the advice from everyone on here and a special thanks to Normcaldwell, it was very much appreciated.

Hey Geo, Good to know that eliminating the post O2 sensor leaks took care of the lean condition.
No leaks, fresh fluids and filters. Part of what keeps this fine vehicle running like a champ.

Really enjoyed driving it after the gaskets and filter went in. Feels so much more responsive and powerful.
